Early Childhood Screenings
Screenings take place on the last Monday of every month. If your child is 3 years old, email Lacey Smith to schedule an appointment for your child to be screened: lsmith@isd166.org

What is Early Childhood Screening?
Early childhood screening is a quick, no-cost check to help identify any possible health or developmental concerns in infants and young children who may need a health assessment, mental health assessment, or educational evaluation. What to bring to the screening.
The paperwork listed below can be downloaded, filled out, and brought with you to the screening. If you need help with any of the paperwork please bring it with you and we are more than happy to assist you at your appointment as well. If you are needing an interpreter please let us know so we can provide you with one for your appointment.
